- Fill in the form electronically, sign electronically, and email to Claremont Senior Program at JoslynCenter@ci.claremont.ca.us.
- Print waiver, fill it in with a pen, scan it and email to Claremont Senior Program at JoslynCenter@ci.claremont.ca.us.
- Print waiver, fill it in with pen, and snail-mail it to: Joslyn Center, 660 N Mountain Ave, Claremont, CA 91711
